Schoettler Book Voucher Request Form

                                                                                                  
This book voucher is distributed the first (1st), second (2nd) and third (3rd) Wednesday for Fall, Winter and Spring quarters. During the Summer quarter, the book voucher is distributed the first (1st) and second (2nd) Wednesday of the Summer quarter.

Eligibility Criteria and Procedures
Foothill and De Anza adhere to the criteria for eligibility for Schoettler Books / Supplies funds established in 2003. To be eligible for Schoettler Book Voucher students must:
  • Have a physical or mobile disability,  or a chronic illness that will lead to a physical disability in the future. Students must have verification of a physical or medical disability in their DRC file, prior to applying for any Schoettler Book Voucher. Disability Resource Center counselors meet with students quarterly, to verify disability documentation, evaluate academic progress, financial aid status, academic progress, and complete Schoettler Book Voucher applications.
  • Make progress towards their goals, as reflected by their GPA (at least a 2.0 cumulative GPA) and demonstrate a history of completing enrolled courses.
The Schoettler book voucher may be used to augment costs for necessary educational materials not covered by book vouchers from other programs.

9. Terms & Conditions

•         Valid through the third week of quarter.
•         Students may apply for one stipend per quarter and may receive no more than 3 stipend awards during their educational career in the District.
•         Purchases may be for class books and supplies only, and prelimited to those specified on this voucher. No duplicate textbook purchases.
•         No cash back for unused amount.
•.        No cash refunds on items purchased with this voucher.
•         Return of any item purchased with this voucher is subject to the posted return policy of the Bookstore.
•         Not replaceable if lost.
•         This voucher may not be used to purchase books or supplies for another person.
•         Purchases are for the term of enrollment at Foothill College only.
•         Books must be returned to the Bookstore for any class dropped by the second week of the quarter;
•         Books must be returned to DRC for any class dropped on or after the second week of the quarter.
•         Bring the completed textbook and supply list to the DRC office by the end of the second week of the quarter.
•         Supplies permitted: paper, notebooks, writing equipment, batteries, backpacks, computer print cartridges, media storage, recording devices, calculators.
•         No IPOD or other electronic equipment, gift Items, clothing, food, or purchases, which would normally not be reasonably associated with course curriculum.

All books and supplies purchases are subject to audit.

VIOLATION OF ANY OF THE ABOVE TERMS WILL RESULT IN IMMEDIATE REQUIREMENT TO REIMBURSE DRC FOR THE TOTAL AMOUNT OF BOOKS & SUPPLIES PURCHASED WITH VOUCHER AND WILL EXCLUDE INDIVIDUAL FROM FUTURE SCHOETTLER SCHOLARSHIP ELIGIBILITY. *This question is required.
