Nomination for the Helping Hearts Lifetime Service Award
Nominate a SCAI Fellow who has provided outstanding service to SCAI over the course of his/her career for the SCAI Helping Hearts Lifetime Service Award. This is your opportunity to recognize a colleague who has demonstrated substantial and consistent service to the Society for over 25 years and has:
- Provided a moral and ethical example for others
- Strived for excellence in patient care
- Dedicated himself / herself to the highest professional standards

The nomination letter should explain why the nominator considers the candidate worthy of the Helping Hearts Lifetime Service Award. The letter should be no longer than two pages and be addressed to: Chair, c/o Stephanie Colbert, SCAI Credentials Committee, SCAI, 1100 17th Street NW, Suite 330, Washington, DC 20036-4628 USA.
3. The CV of the Nominee*:
