Once the form is complete, send one final copy electronically to the FMIGS office at fmigs@aagl.org. The applicant is responsible for the accuracy of the information supplied in this form. Incomplete applications, including incorrect or missing signatures, will not be accepted. Please review your application before submission. After submission, no additional documents nor changes will be accepted.
 
The initial application should be submitted simultaneously with the application fee of $350.00. The application fee is non-refundable. Deadline to submit application is June 15, 2018.
