Consent to Participate in Research


You are being asked to participate in a research study.  Before you decide if you want to take part in this study, you need to read this Informed Consent form so that you understand what the study is about and what you will be asked to do. This form also tells you who can be in the study, the risks and benefits of the study, how we will protect your information, and who you can call if you have questions. Please ask the researcher to explain anything you don’t understand before you make your decision.


The Flood Survey is a survey designed to collect information about the location in which you live, how prepared you are for a flood, flood insurance, and your suggestions for improving flood preparedness and response.  The study is being conducted by RTI International, a research organization located in Research Triangle Park, North Carolina. The purpose of this study is to better understand how people who reside in flood zones think about floods, flood preparation, and flood insurance.


If you agree to participate, you will be asked to complete a brief survey about several topics related to your experiences living in a flood zone. You will be asked questions about your decision to purchase or not purchase property, potential damage as a result of flooding, flood insurance, and your thoughts on the best sources of information pertaining to flooding.

Study Duration


Your participation in the survey will take about 15 minutes.



There are no direct benefits to you from participating in this study. This study may benefit those affected by floods in the future through a better understanding of the challenges associated with living in a flood zone and better information delivery in the event of an emergency.

You will not be paid to participate in this study. 



Many precautions have been taken to protect your information. Your name will not be recorded and your address will be used only to verify the type of flood zone in which you reside. Your address will not be recorded in the electronic data set or linked to your responses.

We will not contact you in the future.

Your Rights

Your decision to take part in this research study is completely voluntary. You can refuse any part of the study and you can stop participating at any time. You can refuse to answer any question. If you decide to participate and later change your mind, you will not be contacted again or asked for further information.
