Canadian Allergy/Anaphylaxis Survey

Introduction

The Allergy/Asthma Information Association, a Canadian not-for-profit, is conducting a survey about severe allergies and anaphylaxis. This survey will take approximately 5 to 10 minutes of your time. The survey results will be used to help educate Canadians about allergies and anaphylaxis and may be used as part of a public relations awareness campaign in 2011.

Note: Many of the survey questions below apply to people with food allergies or anaphylaxis or those who have a family member with allergies or anaphylaxis.

Definition: Anaphylaxis is a severe allergic reaction that occurs in some people when their body reacts to certain allergy triggers, such as food (peanuts, treenuts, shellfish, eggs, milk, fish, soy, sulphites, etc), insect stings, latex, medications, or exercise.

Responses to this survey are considered confidential and therefore individual responses will not be released, shared, or published. Rather survey results will be reported in aggregate data sets.
