Equipping Survey from the Equipping Church Network

As church leaders in the Charis Fellowship, we have the responsibility of equipping God's people for ministry (Eph. 4:12). This survey has been designed by the Equipping Church Network to assess the intentionality and fruitfulness of our equipping efforts. We estimate that it will require 5-7 minutes of your time.
