The consequences of fatigue in the maritime industry are more prominent today than ever before.
This questionnaire hopes to identify whether fatigue is regarded as a serious issue among Officers of the Watch.
Many incidents including collisions, groundings and environmental disasters have resulted from officers being fatigued; take for example the Exxon Valdez oil spill disaster in 1989.
With your participation this questionnaire will increase awareness and help deal with this important issue.
This questionnaire is completely anonymous. All results will be used solely for the purposes of the author’s dissertation and associated publications and will not be relayed to any third parties.
