2018-19 PG 5 Pay for Performance Program "Safety Across the Board" Storyboard

Hospital Improvement Activity

What are you doing to increase safety across the board?  Please utilize your data to identify an area of opportunity within the HIIN scope of work, to develop and implement a plan to drive improvement.  This activity will be highlighted in a storyboard format upon completion.

 

Sample questions to consider when choosing your improvement:
What type of harm are you actively trying to reduce and what is your approach?
What are you doing to reduce your hospital's readmission rates?
How are you trying to reduce health disparities within your community?
How are you engaging patients & families in your quality/safety improvement work?
How are you making your organization more highly reliable?
What activities are you doing to improve your hospital's culture?

 
General Guidelines:
  • The improvement activity could have started as early as the beginning of the GLPP HIIN contract (Sept 2016 – Sept 2018).
  • Storyboards must be submitted through the online template to receive the full 30 points for the P4P program; other versions will not be accepted.
  • You can choose to share photos or a data graph in your storyboard if you would like, but it is not required.
  • MHA Keystone will electronically display the storyboards.
  • Hospitals must individually submit their unique storyboards by November 1, 2018 for credit; system submissions will not be accepted.

Along with the storyboard posters being digitally displayed, several hospital teams may be selected to present their storyboard during a meeting, in a short presentation to discuss their work with attendees.

Please use these general guidelines to gather the information needed for the storyboard. All content must be entered into this online survey.


Please note: The online survey is designed to be filled out all at once -- you will not be able to save an uncompleted survey and log in later to finish. Please have your answers ready before you begin the online survey, and allow sufficient time to complete the survey in one sitting.

Character limits will be imposed on each section so as not to surpass the parameters of each electronic poster. You can view the limits for each area in the online survey.
