Evaluating the Attitudes Toward Seeking Professional Help Scale Consent Form
I understand that I must be 18 years of age or older to participate in the research project entitled “Evaluating the Attitudes Toward Seeking Professional Help Scale” I understand that Chelsea Walljasper and Dr. Jeff Elison, both of Adams State University in Alamosa Colorado, USA are the primary researchers for this project.
I understand that the purpose of this project is to explore the reliability and validity of ATSPHS in terms of potential international differences. I understand that I will be asked questions about my gender, age ethnicity, and attitudes towards help-seeking. I also understand that no procedures in this project involve any experimental manipulation or deception. I also understand that all responses are anonymous and confidential.
I understand that my participation in this project is completely voluntary and will take about 10-15 minutes of my time. I understand that the risks associated with this research are minimal and will not exceed what might be experienced in everyday life.
I also understand that only Chelsea Walljasper and Dr. Elison (University Faculty Professor for PSYC 345) will have access to the raw data collected in this project. I also understand that all responses associated with this project will remain strictly confidential. All responses will be stored in a password protected computer. The data will be deleted after five years.
Chelsea Walljasper (chelsea.walljasper@gmail.com) is the student researcher on this project and will answer any questions concerning the procedures of this projects and will provide aggregate results upon request. In addition, Dr. Jeff Elison (jeffelison@adams.edu 208 Edgemont Blvd, Alamosa Colorado, 81101, USA), is the responsible faculty advisor on this project can also answer any questions
I understand that I may discontinue participation in this project at any time. In addition, I understand that I may request a copy of this consent form by emailing or mailing the researchers listed above.
I understand that I am making a voluntary decision whether to participate in this research project. I understand that by checking this box, I am providing an e-signature that indicates I have read the above information and that I have decided to voluntarily participate.
