Screening adult patients for adverse childhood experiences: A physician survey

Welcome. Because you are a physician, we invite you to participate in our study “Screening adult patients for adverse childhood experiences.”

The goal of our study is both to educate you on the topic and to gather information on your knowledge and opinions about screening adult patients for adverse childhood experiences. Participating in this study will be simple and brief and has been designed to be a useful learning experience. You would only have to answer a short questionnaire which takes approximately 3-4 minutes.

Your participation is entirely voluntary, and we will deeply appreciate your collaboration. The answers you provide will be kept anonymous and confidential. The only identification we ask is your age, gender, specialty and years of practice. The results of your survey are anonymous and we do not have a mechanism to link the completed survey back to you.

By participating in our study, you provide us with your consent to use your non-identifiable, aggregated responses for research purposes.

Those who participate will have the option to participate in a draw for one of two prizes of $200 each.
