Questionnaire

The purpose of this questionnaire is to measure differences between religious and scientific belief. The first few questions of this survey ask for details of any degree education you may or may not have and of your religious or spiritual belief(s). The rest of the questionnaire lists a series of statements about various topics. Read each statement and decide whether you agree or disagree with each statement as follows: 1 - Disagree Strongly 2 - Disagree Moderately 3 - Disagree Slightly 4 - Agree Slightly 5 - Agree Moderately 6 - Agree Strongly Mark the alternative that best describes your opinion. There are no right or wrong answers so do not spend too much time deciding on an answer. The first thing that comes to mind is probably the best response. Be sure the number on the answer sheet corresponds to the number of the statement to which you are responding. There is no time limit, but work as quickly as possible. All data gathered will not be passed on to any third party and anonymity will be protected under data protection legislation. Participants names are only gathered for the duration of the experiment, with all personal data being destroyed by March 2011. At any point you may withdraw from the survey and should you wish your results be discounted from the data after completion please contact Michael at micw778@yahoo.com.
