Nomination: 2018-19 District Wellness Advisory Council Student Voice Committee

In Spring 2017, the New York City Department of Education formed its first District Wellness Advisory Council to provide the NYC DOE's Office of School Wellness Programs with ongoing feedback, review, and support related to physical education, physical activity, health education and wellness programming in New York City public schools. The District Wellness Advisory Council is a voluntary advisory group that includes students, parents, teachers, school administrators and staff, and representatives from other City agencies and community organizations

We are currently seeking nominations for high school students who will serve on the Student Voice Committee of the Council for the 2018-19 school year. Students who participate in the Committee will have an opportunity to share their perspectives on what health, physical education, and other wellness programming look like in NYC schools and how to ensure all schools have quality health and physical education programs.  Students will provide direct feedback to the District Wellness Advisory Council as well as plan and implement district-wide wellness campaigns, such as a to-be-determined health promotion awareness campaign, with the Office of School Wellness Programs and Council.

Students should:
  • Have a demonstrated interest in health and wellness, including but not limited to topics covered in health education and physical education.
  • Be entering or enrolled in a NYC high school for the 2018-19 school year.
  • Have demonstrated leadership skills.
Students will be asked to:
  • Participate in quarterly District Wellness Advisory Council meetings during after-school hours
  • Participate in bi-weekly or monthly Student Voice Committee meetings during after-school hours
  • Share their perspectives in solution-oriented ways that will advance health education, physical education and wellness-related programming in NYC schools.
  • Advise on and plan initiatives as needed, depending on their capacity.
  • Provide recommendations to other District Wellness Council Advisory Council groups.
  • Participate in leadership building opportunities as available.
Students will receive:
  • An opportunity to make a difference on school health in NYC and for the lives of their 1.1 million peers.
  • A chance to meet health, wellness and education professionals from across NYC.
  • Development of leadership skills.
  • Opportunities to meet other young people from across NYC and collaborate with them on important health initiatives.
All student nominees will be reviewed by members of the Office of School Wellness Programs.  Students will be selected based on their demonstrated leadership skills, involvement in their school community, and personal statements.  If selected, students will be asked to serve on the Student Voice Committee through the 2018-19 school year and will review student nominations for the 2019-20 school year.  Students will need parent/guardian permission in order to participate in the Council.

If you have identified a student who meets the above criteria and is interested in being a part of this Council and Committee, please work with that student to submit this nomination form. Please note that questions 4 and 5 should be answered by the student, demonstrating their commitment to this work. Please only submit one nomination per school.

The application deadline has been extended to Friday, November 30th, 2018.
