AK-PA Co-Sponsorship Application

Thank you for your interest in working with the Alaska Psychological Association (AK-PA) to provide Continuing Education credits for psychologists who attend your upcoming training event! Please complete this form and submit it to AK-PA for review at least 45 days prior to the event date.

Please note the following:

You can use the Save and Continue feature to save your progress and return to complete the application form later. You'll enter your email address and a password you create, and use the same link to return to the form.
A response is required for all questions in the application form. You will receive a notification if you have left any questions on a page unanswered, but will still be able to move between the pages of the form. When you press Submit, you will be presented with a review screen that will allow you to ensure you have responded to all of the questions.
