I am currently working on my senior capstone project in my major at Pitt, Health Information Management, and was wondering if you could take a moment out of your day to answer this short questionnaire. Your responses will provide me guidance on the current face of healthcare and what we can work towards in the future in the field of Health Information Management.

1. Does your department or facility use an electronic health record system?
2. Are your patient medical records paper or electronic?
3. On a scale of 1 to 5, 1 being not important and 5 being extremely important, how important do you believe it is to have a correct and complete patient medical and surgical history?
5. If your department or facility uses an electronic health record system, do you find that the information in your electronic health record system is up to date?
6. If your department or facility uses an electronic health record system, do you personally enter information into the electronic health record system?

8. If you do personally put information from the patient medical record or clinical documents into an electronic health record system, on a scale from 1 to 5, where 1 is not time consuming at all and 5 is extremely time consuming, how time consuming do you find this to be?
9. Does your department or facility employ a medical record abstractor?
10. If you have an electronic health record system, would you find it more useful to have an additional employee to perform reconciliation tasks into the electronic health record system or to have software to present the information to you in a way that makes it easy for you to perform these reconciliation tasks?
11. On a scale from 1 to 5, where 1 is do not understand at all and 5 is completely understand, how well do you understand the meaningful use requirements under the American Recovery and Reinvestment Act passed by President Obama to aid in the use of electronic health record technology?
12. Do you find it difficult to exchange information between healthcare providers to provide the best healthcare to the patient?
13. Do you fear that you lose the narrative of useful information from a patient's medical record when the clinical report information is abstracted into an electronic medical record?
