YOR California Application

Welcome

Welcome to the YOR California online application!

The application process is straightforward, but it does require time to prepare and complete all required documents. It also involves identifying a lead agency and deciding whether to collect letters of commitment from partners. Therefore, you are encouraged to begin the application process early and submit your application before the deadline, just in case you need help with any of the application required components.

Application deadline: June 7, 2019, 4 p.m. PT
Late applications will not be accepted.

Please fill out this online application and upload your attachments where indicated. You will be able to work on your application, save your work, and return to it at your convenience. However, once submitted, no further changes can be made to your application or attachments.

To save a partially completed application, click on “save and continue later” located in the top right corner of this online application. Provide your email address and click “save.”  You will receive an email from SurveyGizmo providing a link that allows you to return to the application at any time. Once you receive this link, your online application will auto-save each time you complete a new section. Upon submission, you will receive an email confirming your application’s receipt.

Additional application instructions are located in the YOR California Request for Applications (RFA).

For additional questions or assistance, please email YORCalifornia@ahpnet.com.

Thank you for your interest and commitment to improve and expand access to prevention, intervention, and recovery services, as well as access points to Medication-Assisted Treatment (MAT), for youth (ages 12-24) and their families.
