This survey will take about 10 to 15 minutes to complete. It will ask about your perceptions about underage drinking among high school age youth (ages 14-19) in your community. There should be no risk to you in completing the survey. The survey is anonymous—we are not asking for your name or any other identifying information that can link your answers to you.
Your participation in this survey is completely voluntary. You may choose not to answer a question and you may quit the survey at any time. There are no right or wrong answers.
- If you are unsure how to respond to any questions, please mark your best guess. If you feel strongly that you do not know which response to provide, you may mark “don’t know.” (this option is not always available, since we are interested in your best guess)
- Some questions ask what you think about alcohol and your child. If you don’t have children, please answer the question as if you did have children.
- When we ask about drinking alcohol, we mean more than a few sips.
