Disaster Relief Survey

Informed Consent

1. Please read the following material that explains this research study.

Your consent (below) indicates that you have been informed about the study and that you want to participate.

We want you to understand what you are being asked to do and what risks and benefits—if any—are associated with the study. This should help you decide whether or not you want to participate in the study.

The Fire and Disaster Relief Community Media Project will use the insights and experiences of individuals impacted by local disasters to develop an innovative model for sharing information both during and between disasters. This brief online survey will assess your needs during a natural disaster. The survey should take no more than 10-15 minutes of your time. The survey will also ask about your willingness to participate in a series of in-depth interviews to understand the experiences and changing needs of the fire victims, responders, aid agencies and media outlets. The project will help outside communities to understand how a community responds to - and learns from - a disaster.

Your participation is voluntary and is not compensated. There are no anticipated risks or dangers associated with this study protocol.
