Thank you for your interest in joining the Healthy Start Breastfeeding Cohort! 

The purpose of the Healthy Start Breastfeeding Cohort is to convene Healthy Start grantees – who are prepared to and interested in improving and building out their breastfeeding support and education activities – and support them in meeting the Healthy Start breastfeeding benchmarks.

Participants will: 
  • Receive TA and support that will enable grantees to meet the Healthy Start breastfeeding benchmarks:
    • Increase proportion of Healthy Start child participants whose parent/caregiver reports they were ever breastfed or pumped breast milk to feed their baby to 82%
    • Increase proportion of Healthy Start child participants whose parent/caregiver reports they were breastfed or fed breast milk at 6 months to 61%
  • Participate in capacity building activities that will align with Healthy Start Approach #4:
    • Assuring impact and effectiveness through workforce development…
  • Engage with and learn from one another and subject matter experts
  • Co-create breastfeeding education materials for the greater Healthy Start community

The following application will take between 20 and 30 minutes to complete. Please note:
  • Applications will be accepted until COB Friday, May 29. Any applications accepted after this deadline will NOT be considered.
  • You may NOT fill out this application on someone else's behalf (including for staff who you plan to onboard soon).
  • Please ONLY complete this application if you are prepared to commit to participating in the Breastfeeding Cohort from June 2020 through January 2021. 
  • You must receive approval from your Healthy Start Project Director and your direct supervisor (if different from your PD) before submitting this application.
  • If you fill out this application and are selected to participate in the cohort, but end up leaving your Healthy Start project, your position in the cohort cannot be transferred to another employee.
The TA & Support Center will notify applicants whether they were accepted into the cohort or not by Friday, June 5. The first webinar for the Breastfeeding Cohort will be held the following Thursday, June 11.
