Introduction

This is a Performance Review survey for one of your co-workers. The results of this survey will be provided to the person in question and his/her managers. Please read this whole page before starting the survey.

Purposes of Performance Review and survey:

  • To help in assessment of people on Probation Period.
  •  To find the strengths of the employee and communicate Team's appreciation of these strengths.
  • To give constructive feedback for areas where we see potential for improvement.
  • To let the employee know how he/she is perceived by the Team, letting him/her 'feel the ground'.
  • To stimulate and help in self-growth of the employee.
  • To help in developing employee's career.
  • To communicate what is important for the company.

Guidelines:

  • Please spend at least 15-30 minutes writing your constructive feedback - I'm sure the person in question will appreciate it.
  •  The survey is divided into one-page sections covering different areas of feedback.
  • In each section there are several points - angles from which you can look at your mate's performance. The role of each point is to try to inspire you to give interesting feedback. However, most of the points will not inspire you. This means that:
    • Do not provide comments for all the points! Please comment only if you have something interesting to say. It is OK to leave most of the points empty.
    • Your comment doesn't have to be strictly related to the point.
    • Points are not strict, may cover overlapping areas.
  • However, please spend at least 10 seconds contemplating each of the points, and consider:
    • Proficiencies you can highlight,
    • Constructive suggestions for improvement.
  •  Provide examples that justify your opinions.
  • Be sensitive.
  • Make it clear if you don't want some of your comments to be shown to the person in question.
  • The survey has almost 40 points.
