This form will indicate whether you want to work in NYU America Reads / America Counts in the next academic year, and, if so, whether you want to continue to work in the same school. If you do not wish to work with America Reads in the future, or are ending your NYU career, submitting this form will keep your record clean in the event of future job references, etc.
