2014 John Philip Sousa National Honors Bands - New England Region TEACHER RECOMMENDATION FORM

1. TEACHER RECOMMENDATION FORMPlease Note… This form is for TEACHERS ONLY! Please submit one recommendation for EACH STUDENT that you nominate. Please use proper UPPER and LOWER CASE LETTERS where appropriate. Please have each student fill out own APPLICATION FORM (also online) for nomination purposes. There is no limit to the number of students submitted per school, although the group is clearly looking for the very best band musicians in grades 7, 8, 9, 10, 11, and 12, and the selection process will reflect that aim.

Groups are being selected from those who have already applied, although applications may still be accepted during this process. Students will receive notice of acceptance by February 15, 2014, along with supporting paperwork for housing, medical forms, etc. Registration fee for accepted students will be $195.00 per student for those staying at the Howard Johnson's Plaza, Portland, ME, and $125.00 for students living within a reasonable commuting distance of Gorham, ME.
