ABC Corporation has partnered with SurveyGizmo to conduct an employee engagement survey. We are doing this to identify strengths as well as opportunities for improvement.
Please familiarize yourself with all instructions before completing the survey. In order to provide you with valid and reliable data that is free of contamination, it is important that you follow all instructions.
Please answer the survey honestly and completely so that the results will be a constructive management tool for the organization.
Mark the circle of the response that is closest to your opinion. Try to think of how you usually feel about each question. Mark the "Does Not Apply to Me" response only if you do not have an answer.
Toward the end of the survey you are invited to write your opinions on specific questions. Your comments will be entered in the final report exactly (verbatim) as you write them and will be reviewed by management.
Confidentiality
To ensure your confidentiality, your responses will return directly to SurveyGizmo. No one at ABC Corporation will ever see your individual survey responses.
Data will be reported in aggregate format and sorted by demographic groups. SurveyGizmo will only report on groups with five or more participants. If there are fewer than five respondents, SurveyGizmo will combine that group's data with a larger group.
