Thank you for taking the time to participate in Generations United Outreach Survey. This survey will provide Generations United with information about active intergenerational programs and shared sites around the country. We will develop a database for programs so that you and other intergenerational practitioners can learn about innovative and promising practices.

This survey should take approximately ten minutes or less to complete. We want to promote your good work, and can do so with your feedback. If you have any questions or concerns, please contact us at 202.289-3979 or gu@gu.org.

4. Do you partner with any other organizations in your community? (Check all that apply)
5. What age ranges does your program serve? (Check all that apply)
6. Do you intentionally evaluate your program? (This can mean both formal and informal evaluation)
7. If yes, primarily through what means? (Check all that apply)
8. If yes, would you be willing to share your evaluation method and results with Generations United?
