Trainee Form

Consent form

I agree to take part in a research study titled "Factors contributing to cultural discussions with Asian international counseling trainees: cognitive style, theoretical orientation, supervisory style, multicultural competency, and acculturation", which is being conducted by Yi-Chen Wu, Counseling Psychology at University of Georgia, under the direction of Dr. Edward Delgado-Romero, Counseling Psychology at University of Georgia.

The purpose of the study is to find effective ways to help Asian international counseling trainees to talk about cultural issues in supervision. By participating in this 20-30 minutes online survey study, I have the opportunity to contribute to the multicultural supervision area in psychology field and help future supervisors provide effective clinical training to Asian international counseling trainees. I understand that I will be required to nominate a supervisor whom I worked with for at least 3 months. This supervisor will be contacted through the email address provided by me and asked to participate in the study to give their perspectives about how they provide supervision for Asian international trainees.

No individually-identifiable information about me, or provided by me during the research, will be shared with others unless required by law. My supervisor will be informed that one of their Asian international trainees nominated and gave the researcher their contact information. While the information I provide for the study will be linked to the information provided by my supervisor, my supervisor and I will not have access to each other's results. Internet communications are insecure and there is a limit to the confidentiality that can be guaranteed due to the technology itself. However once the materials are received by the researcher, standard confidentiality procedures will be employed.

All participation is voluntary. I can refuse to participate or stop participating at any time without giving any reason, and without penalty or loss of benefits which I am otherwise entitled. I can ask to have identifiable information related to me returned to me, removed from the research records, or destroyed. There are no more than minimal risks anticipated; however, some individuals may feel slightly uncomfortable answering questions about their cultural competencies. Thus, I understand that I may skip any questions I am uncomfortable responding to.

Ms. Wu or Dr. Delgado-Romero will answer any further questions about the research, now or during the course of the project. Ms. Wu can be reached at yicwu@uga.edu or (865)824-8951 and Dr. Delgado-Romero can be reached at edelgado@uga.edu or (706)542-1812.

By clicking "I agree" below indicates that the researchers have answered all of my questions to my satisfaction and that I consent to volunteer for this study.
