Repeat Puppy Raiser Enrollment Form

To be considered a repeat raiser, you must have raised or continued sitting puppies within the past two years. If you don’t meet these qualifications, we ask that you fill out a Puppy Raiser Application. Your ability to repeat raise is at the sole discretion of Dogs Inc.

1. Have you raised or continued sitting puppies for Dogs Inc in the past two years? Raisers who have not been active in the past two years will be directed to complete the Puppy Raiser Interest Form. *This question is required.
