This survey instrument assesses your behavior under normal conditions, as well as during times you are under stress. The more you understand yourself and your behaviors the better you will be able to make those changes that will benefit you and help you achieve success in your endeavors.

Please answer ALL of the following questions—even though some of them may reflect behaviors that you exhibit only rarely.

Directions

Using a total of 10 points for each of the four-item sets below, divide these 10 points in any way you wish between the four choices. You will want to assign the highest number of points to the items you feel most typically describe you, and less points for those items that are only a little like you. You may even score 0 points if you feel that any of the items do not apply at all.

Please be sure your answers in each set add up to 10 points and please answer every item, even if you find various sets difficult to score.
