How to Create a Survey with Informed Consent

We know that getting the consent of survey participants to take a survey can be vital to some of our customers, so we thought we would share a few quick tricks on how to do this.

Easy Option #1

  1. On the first page of your survey, include only one question asking for your survey taker’s consent. For this option use a Checkbox Question with the option as, “Yes, I consent” (or something along those lines) and make sure the question is Required. We suggest removing numbering for this question as well.Survey Consent Screen Shot

When you make the question required, you’ll only allow participants who manually agree to take the survey into the survey and everyone who tries to get in without checking the box will show up as either a Partial response or Abandon in your Survey Overview.

Easy Option #2

  1. On the first page of your survey, include only one question asking for your survey takers consent.  For this option use a Radio Button Question with the answers Yes or No. We suggest removing numbering for this question as well.
  2. For anyone who checks No, you’ll want to disqualify them and no longer allow them to take the survey.  To do this click the gear icon (Simple Page Jumping) next to the question in the Question Editor and set it to skip to the Thank You page.Basic Page Jumping

Doing this will allow you to see how many people are consenting and how many are not, because when they hit the Thank You page their response will be counted as complete.

Those are the two easiest ways to create a survey where the participants check that they consent.

Note: There are more advanced techniques including adding a Redirect Browser Action and sending the “no” participants to another page completely. You can also add Advanced Page Jumping & Logic and redirect your “no” participants to a disqualification message, but using Simple Page Jumping is the easiest and quickest way to accomplish this.
