1. The survey title should clearly summarize the survey objective
2. Avoid using “and” or “or” in your question – it may be two questions in one and both statements may not apply
3. Be sure to remove those who have already responded from your reminder list
4. When selecting question types consider the time involved for the analysis stage e.g. coding & evaluating open-ended questions
5. Be sure to include your contact information in the survey invitation
