1. Use a different color or font to make instructions distinct from questions, your survey will be easier to follow
2. Open-ended questions should be voluntary, not mandatory
3. Avoid double-barreled questions – make sure each question addresses only one issue or attribute
4. Mention in your invitation a deadline/date to complete
5. Ask yourself “What will I do with that information?” – If you don’t know, delete the question
