Health Assessment Surveys include Health Evaluations, Healthcare Questionnaires and Health Needs Assessments. Theses types of surveys are used to evaluate overall health and health risk factors. They also focus on respondent wants, or needs for health improvements and include ideas of how to implement on these desires.

When it comes to sensitive data, specifically healthcare data, security is of utmost importance. Survey Goal

Before starting your Health Assessment Survey, you will need to clearly define your survey goal. This will help you to stay on track with the questions that you are asking in your survey.

Here are examples of Health Assessment Survey goals:

  • Survey Goal Sample #1: To asses a patient’s physical condition and daily habits in order to provide recommendations to achieve better health and a healthier lifestyle.
  • Survey Goal Sample #2: To determine the appropriate health insurance plan for our company, we will assess overall health and wellness of employees and their families that may be interested in being covered by our plan.

A general mindset when dealing with sensitive data like the type of information that you gather from Health Assessment Surveys, is security. SurveyGizmo provides lots of options and ways to keep your data secure, and help respondents feel confident about filling out your survey:

  • Permissions - Allows you to limit users access to surveys and / or results. If you have sensitive information in your survey, that cannot be viewed by either the survey builder, or other colleagues, you can limit their access with the SurveyGizmo Team-Based Permissions.
  • Secure links - Make sure that your data is secure by setting up secure links to your surveys (also called SSL or https:// surveys). This will encrypt data during the respondent’s survey session.
  • Branded Surveys - A Branded Subdomain is a survey link that includes your own branding along with SurveyGizmo standard branding. Private Domains allow you to completely brand your survey link with no reference to SurveyGizmo. Respondents will have an increased sense of confidence when inputting personal information when they see a familiar name in the survey url.
  • Survey password protection - To add more security and respondent confident, you can add a password to your survey. In order to start taking the survey, the respondent would need to put in the appropriate password.

Survey Questions

The questions that you use in your Health Assessment Survey will be dependent on your survey goal.

One direction you may choose to go is to ask a series of Yes / No radio button questions about the factors you are looking to test. For example, here are two of these types of questions in action:


Another option would be to have respondents evaluate a variety of factors on a radio button 5-point textual scale from Strongly Agree to Strongly Disagree. For example, here are two of these types of questions in action:


Regardless of the specific healthcare focused questions that you ask, and the type of questions you ask you will want to have a section with demographic questions. These questions are useful for segmenting data and are usually located the end of your survey. Some suggested demographic questions include:

  • Gender
  • Age
  • Marital Status
  • Household Income / Individual Income
  • Highest Level of Education Completed
  • Ethnicity
  • Race
  • Employment Status

These are just a suggested start for your surveys. Remember to keep your Survey Goal in mind and make your survey meets those standards.

If you used the radio button 5-point textual scale questions from Strongly Agree to Strongly Disagree, when running a Summary Report, your results for a single question may look like this:

According to this sample data, 18.7 percent of respondents strongly agree with this healthcare question.

If you filter your Summary Report by female respondents, which is 43 percent of the sample data population, you would see that female respondents are more likely to strongly agree with this statement (25.0 percent).

Please note that all data used in these examples is test data and not actual survey data.
