Contact forms are a quick and organized way to collect information from current and prospective customers, as well as obtain feedback from people who visit your website or blog.
Make it easy for people to get in touch with you by embedding a contact form directly on your website or blog. Our standard embeddable survey functionality allows you to embed your contact form in a matter of minutes using one of the following options:
- JavaScript Form Embed- Simply copy and paste in a few lines of code that we supply to make your web form live on your website.
- iframe Form Embed- Designed for those who are unable to use JavaScript but still want to embed a form directly on their website using a simple copy and paste method.
- HTML Form Embed- Embeds the actual HTML directly on your website. This option works well with short, single page forms. With multiple page forms, your respondent will complete the first page on your website then be redirected to SurveyGizmo’s website to complete the remainder of the contact form.
Contact Form Questions
Contact forms are ordinarily very short, sometimes asking the respondent as few questions as name and email address. Unlike many contact forms, however, SurveyGizmo does not restrict the length of the form. This enables you to include as many questions and pages as needed.
Standard contact forms may include a combination of the following questions:

Contact forms give your audience a convenient way to reach you without publicly listing your email address online. Publicly listing your email address on your website leaves your email vulnerable to spammers.
To help safeguard against spammers, SurveyGizmo has enlisted the help of reCAPTCHA. reCAPTCHA is an online program that generates and instantly grades word tests. These word tests portray distorted words and letters that humans have the ability to answer correctly but computers and “bots” do not.

By default, SurveyGizmo stores the data collected from your contact form directly into your account. To increase efficiency, you can use an email auto-responder to alert you via email that a contact form has been submitted. This is ideal for low-volume forms and surveys like contact forms. Auto email-responders give you the ability to instantly view your collected data. This allows you to quickly resolve any issues that may arise or respond to customers and potential customers instantly.
Your contact form can also trigger a response email to be sent to the person upon completing your form. This email can provide a confirmation of successfully completing the contact form or serve as a “thank you for your inquiry or sign-up.” Email auto-responses are completely customizable, allowing you to modify the email message to fit your purpose.
Reporting
Contact forms are generally composed of text box questions. The most efficient way to view the data collected in this way is through a CSV export.
Exporting your data to a CSV (Comma Separated Value) file gives you a highly compatible format to edit, view and analyze your data. CSV files use a universal format for storing spreadsheet data that can be opened using any spreadsheet software as well as a basic text editor.
A CSV export assigns each question a column and each respondent a row. The row headers will contain the questions followed by each individual respondent’s answers displayed one row per respondent.
