Contact Information Question Type

What is this?

Contact Information (displayed in the application as “Contact Group”) is a special question type that allows you to easily collect contact information from a survey respondent including, but not limited to: name, phone number, email address, and physical address.

When would I use this?

This question type is generally used whenever you need to contact the person that filled out a survey after the fact. An example of using this question type would be when you were creating a ‘contact us’ form to embed on your website. You may also want to use this option to find out if the survey taker has changed their address from what you have on file.

How does this Report?

Reporting Example

In a Summary Report, the contact group will show an individual appendix element for each textbox field in the contact group. To save space, there are only two listed below, but in a normal report, each one would be included.

Exporting Example

In the CSV/EXCEL export, there will be a column for each individual textbox field of the contact group. Each column will display the answer options first, then the overall question. If a respondent has left a cell in the question blank, inside the CSV/EXCEL export, it will be represented by a blank cell as well.
