Question Types

The complete list of question types are available to Pro, SurveyGizmo, Enterprise, and Dedicated customers. This section will describe how each question is formatted, edited, and reported/exported.

Textbox

Textbox Questions are Available to All Account Levels.

The textbox question type allows your survey taker to input a short textual answer. This question type is ideal for single word or short sentence answers. If collecting more than 6-8 words, we recommend using the essay question type.

This question type can be edited using our Quick Edit feature.

Number

Number Questions are Available to All Account Levels.

 The number question type asks the survey taker for a numeric value, such as a year or quantity. It contains validation by default that ensures that the value is only a number and contains no additional types of characters.

Email Address

Email Address Questions are Available to All Account Levels.

The email address question type asks your survey taker to input an email address into the survey.

Percent

Percent Questions are Available to All Account Levels.

The percent question type asks the survey taker to enter a percentage. This question type validates the answer as a numeric value and adds the percent sign automatically if it is not input by the respondent.

Date

Date Questions are Available to All Account Levels.

The date question type asks the respondent to provide a date. A calendar icon is automatically provided that allows the respondent to select the date through a point-and-click calendar, entering the date automatically.

Essay

Essay Questions are Available to All Account Levels.

The essay question type is used to ask a respondent for lengthy open-ended replies. It is typically used to allow for additional comments.

This question type can be edited using our Quick Edit feature.

For example, you can ask your respondent why they rated their overall satisfaction with your company as "Extremely Satisfied". Their essay response can add some texture and additional information to help more fully understand their response.  You can determine the response length by setting the maximum number of words that the respondent can input. You can also set the width and the height of the Essay question input box. This question type is great for descriptive comments.

Within our Summary Reports, you will see an appendix element for each essay response. Inside of each of these, the data will display each unique response and the count of how many respondents answered that way. The best way to report on this question type is to use the Appendix element.

List of Textboxes

List of Textboxes Questions are Available to All Account Levels.

The list of textboxes question type creates multiple rows of textboxes with independent labels for each row, all within a single question.

For example, if you wanted to ask respondents for their favorite movie by genre you can provide the labels: Drama, Comedy, Foreign and Western. Respondents can then fill in each textbox with their favorite films. You can define the textbox width (by number of characters) and choose to position the labels on the left or right side of the input box. This question type is great when you are collect short text responses to lists of prompted questions.

Within our Summary Reports, you will see an appendix element for each row of your List of Textboxes. Inside of each of these, the data will display each unique response and the count of how many respondents answered that way.

The CSV/XLS export will assign a column within the spreadsheet for each row header within the list of textboxes. It will display the row headers first, then the question itself. In each individual response row of the csv, you will see what the respondent answered.

Radio Button

Radio Button Questions are Available to All Account Levels.

The Radio Button question type is the most common survey question format. It present the respondent with multiple choices and allows them to select only one answer.

You would use a radio button question if you needed your respondents to choose a specific option out of pre-defined answers. A great example of a Radio Button question would be, "What is your gender?". The answers would be "Female", "Male", "Other", or "Prefer not to answers. Our Radio buttons also allow you to use an other textbox to find out more information.

Another example would be "What is your age?". Instead of having an open text box that they fill in, you can have a radio button that provides pre-defined ages such as "18", "19", "20", etc.

Multiple columns are present for each answer possible within the checkbox question. The answer given is present in the cell if that answer was selected by the respondent.

Dropdown Menu

Dropdown Menu Questions are Available to All Account Levels.

This question type allows your survey taker to select one answer out many from a dropdown box. We also the ability use Multi-Select Dropdown Menus.

This question type is great to use when you have a large amount of answer options and not a lot of space within your survey. This question type is ideal for demographic questions such as selecting a State or Country you live in. If you like the functionality of a radio button, but would like a visual change of pace, you can also use Dropdown Menus to vary up your survey question types.

Multi-Select List: Enabling this option will allow the survey taker to select more than one option from a list, and allows you to have another aesthetic option for choosing multiple answers. Please see Multi-Select List for more information.

Within Summary Reports, the default dropdown menu can be displayed as a pie chart, bar chart, or table. If the unique setting to create a multi-select box is used, reporting should not use the pie chart display type.

The default dropdown menu is single-select, and the export will have a single column of data. However, if multi-select box is enabled, a column will be present for each answer possible. (See Checkbox)

Multi-Select List

The simple Dropdown Menu question also has the ability to convert into a different question type that will allows users to select more than one option from a list. Multi-Select Lists allow you to have another aesthetic option for choosing multiple answers.

 

To access this, add a Dropdown Menu question, name it, then once inside, go to the Formatting tab and click the checkbox for "Multi-Select List".

Multi-Select Lists are very helpful if you have a large amount of answer options, and you feel checking a small checkbox for each would be tedious. Instead, you can use a Multi-Select List using your CTRL (or CMD) key to click anywhere on each option of the list and easily select as many that apply. For example: Instead of creating multiple Dropdown Menu questions to find out which ice cream flavor is your respondents’ favorite, you could use this feature to help your respondents select their top three flavors within one Multi-Select list. This helps you to consolidate your data, and gets your respondents their ice cream faster!

Within Summary Reports, a Multi-Select List will be displayed using a bar or table element. Reporting for this question type should not use the pie chart display type.

Within our CSV/EXCEL exports, multiple columns are present for each answer possible within the Multi-Select List question. In the CSV/EXCEL file, it will display the answer option, then the question in the column headers. The response given is present in the spreadsheet cell if that answer was selected. Otherwise, if the answer was not selected, the cell will be left blank.

Cascading Dropdown Menu (BETA)

Cascading Dropdown Menu Questions are Available to All Account Levels (contact support@sgizmo.com to access them).

We have a preliminary release of an exciting new question type!  Brace yourself for the Cascading Dropdown Question!  This dynamic question allows you to drill down through hierarchical data by using one dropdown menu as a filter into the next. For, example, a very common use is the collection of the survey taker's state and city.  Using this question type, the survey taker's state selection can be used to filter the options in the city dropdown.

If you need your survey takers to select from a very large list, say the university/school they went to, that list can be up to 4000 items long!  Using a Cascading Dropdown Question to filter school by state would reduce the burden on the survey taker by limiting the number of answer options in the dropdown.  By making it easier for the survey taker to locate values, you'll improve your survey taker's experience and, as a result, you'll likely get better data!    

Some additional examples of when you might use a Cascading Dropdown Question:

-  Product Category, Product

-  Car Make, Model, Year

-  State/Province, Cities

-  Sales Region, Sales Representatives

Summary Reports for Cascading Dropdown Questions are not available at this time, you'll need to export your raw data in order to report on this data. Above is an example of the data export. Each column in the export will display the individual dropdown menu options title, then the question.

Image Choice (Single Selection)

Image Choice Questions Available at Personal(Basic) and Above

The image choice question type displays multiple images to the respondent allowing them to select their answer by clicking directly on the images.

While it is beneficial to adjust the size of your images prior to uploading them to your library, you can force a specific size (or force the size of one dimension) by specifying a width, height, or both (1). You can also decide how many images (2) will be displayed in a row before a new row is created. This assumes that the width of the survey supports that number of images in a single row.

List of Dropdown Menus Questions are Available to All Account Levels.

The list of dropdown menus question type includes multiple rows of individual dropdown menu questions, each row with a unique row label. All of the answers available within the dropdown menus are the same for each row.

Force no repeats requires that there at least as many answers within the dropdown menu as there are rows in the list, and the respondent can select any particular answer only once within the entire question.

Individual rows within the list can be graphed using a bar or pie chart, however to display the entire question, the table format will display all the data from each row within the list of dropdown menus.

Likert Scale

Likert Scale Questions are Available to All Account Levels.

The likert scale question type asks the respondent to select a position on a scale of choices, such as very unhappy to very happy. These choices are shown horizontally, with labels above or below each choice. Typically, the reporting value is numeric for analysis purposes.

Unique to likert scale questions, a label can be placed to the extreme left or extreme right of the entire horizontal scale (1). The labels for each answer is placed on top of the radio button by default, but checking the Position Labels on Bottom (3) option will switch them to below the radio buttons. If Flip Scales Randomly is checked (2), the answers will be randomized while maintaining order (e.g. 1,2,3,4,5 or 5,4,3,2,1) when displayed to the respondent, maintaining the proper saved value for reporting.

Drag & Drop Ranking

Drag & Drop Ranking Questions are Available to All Account Levels.

The drag & drop ranking question type asks the respondent to rank a list of choices by dragging and dropping the choices in order of preference. The interactivity of this question type makes it a less screen reader accessible question type, and SurveyGizmo offers an alternate fully accessible version
with the table ranking question type. (See Table Ranking)

Within Summary Reports, the Drag and Drop Ranking question can only be represented in a table element. The eeport will show you three columns. The first will be your answer options. The second will be your Total Score, and your third will be your Overall Rank.

The total score is the weighted calculation. Items ranked first are higher than the following ranks. The score is the sum of all the weighted rank counts put together. For example, if the report above, because there were only 5 options, the weighted sum for an option that was placed in the first position (1) would be worth 5.

It would come out as the following:

If you had 7 answer options, then Rank 1 would be given 7 points (so and and so forth).

Table Ranking

Table Ranking Questions are Available to All Account Levels.

The table ranking question type asks the respondent to rank a list of choices by selecting their numeric rank from a table of radio buttons, where each column is restricted to a single answer across all rows. **NOTE**: This format is fully screen reader accessible.

If you decide half way through creating a Drag and Drop Ranking question that you no longer want an interactive question, but would prefer a table, you can switch it over to a "Table/Grid" option.

Continuous Sum

Continuous Sum Questions are Available to All Account Levels.

The continuous sum question is used to gather a breakdown of a sum (or 100%) from the respondent. This question type is used to see how a larger value, such as a budget, is allocated over several choices. The continuous sum automatically tallies a total of all the values in each textbox.

The continuous sum question supports the following settings:

Basics Tab:

  • Within Answer Options: Fixed Position (This answer will stay where it is despite randomization)
  • Within Answer Options: Default Value

Validation

  • Max Total
    • The answers for this question *must* equal the max total
    • Hide Maximum Value
  • Max Total Alert Message

If the answers given should be required to sum equal to or less than a particular value, that can be set as the Max Total (1) which is visible by default, but can be hidden (2) from the respondent. If the respondent should not be able to continue unless the Max Total is met exactly, check option (3). If they are over the Max Total, a custom message can also be set (4).

Within our Summary Reports, the Continuous Sum question is compatible with the table element. It will display the Answer options, then the Min, Max, Std Dev, Average, Sum, and Total# of responses in the chart.

Within our CSV/EXCEL exports, similar to the Summary report, row from your List of Textboxes will have it’s own column header. It will display the row header, then the question.

**Note**: The total that is displayed for the respondent is not available as part of reporting.

Star Ranking

Star Ranking Questions are Available to All Account Levels.

The star ranking question asks the respondent to interactively rate criteria of different categories defined by the row and column headers. Each star represents the equivalent numeric value for that rating (e.g. Four star rating will report a 4).

The most common use for a Star Ranking question is to rate products such as books, or movies. However, you can also rate things like customer service and satisfaction. In general, you would want to use the Star Ranking question when you’re trying to put a positive spin on whatever you’re rating.

(1)The number of stars used for rating can range from 1 to 10 stars.
(2) Check the Pre-population Stars option to enable a color change for stars that are preset via pre-population from a custom scripting action. (See custom scripting)

The star ranking question can be displayed as a pie or bar chart if viewing any individual row within the table. However, the most complete view is to use the table format to view the ranking comparison of all rows and columns, including highlighting of the highest rankings.

Table of Checkboxes

Table of Checkboxes Questions are Available to All Account Levels.

The table of checkboxes question allows respondents to select one or more matching criteria (columns) about each item listed in the left most column (the row headers). This question type is a convenient way to display multiple checkbox questions (each row) that share the same possible answers (columns).

A table of checkboxes can be displayed as a pie or bar chart if using an individual row within the table. However, to see all of the data in a single element, the table display will show the complete results within a summary report.

The CSV/XLS export will assign a column within the spreadsheet for each row and column combination within the table of checkboxes, displaying the reporting value in the column if the respondent selected that particular value within that particular row.

Table of Radio Buttons

Table of Radio Buttons Questions are Available to All Account Levels.

The table of radio buttons question allows respondents to select one of the possible criteria (columns) about each item listed in the left most column (the row headers). This question type is a convenient way to display multiple radio button questions (each row) that share the same possible answers (columns).

A table of radio buttons can be displayed as a pie or bar chart if using an individual row within the table. However, to see all of the data in a single element, the table display will show the complete results within a summary report.

Table of Dropdown Menus

Table of Dropdown Menus Questions are Available to All Account Levels.

The table of dropdown menus question allows respondents to select one answer from each dropdown menu to address the criteria specified by the row and column alignment. The entire table contains the same list of answers within every dropdown menu. To have different answers in each column's dropdown menu, you will need to use the custom table question type. (See custom table)

Table of Textboxes

Table of Textboxes Questions are Available to All Account Levels.

The table of textboxes question allows respondents to provide an open answer for each specified row and column alignment.

For example, if you want to know what a respondent had to eat for each meal of the day for a week, you can set up the days of the week as the row headers (Monday, Tuesday, Wednesday, etc.) and you can set the meals of the day as the column headers (breakfast, lunch, and dinner).

Respondents can then type in what they ate each day for all three meals. You can define the width of the textboxes by assigning the number of characters. This question type is great for multiple questions that are asked for different scenarios. You can also use the formatting tab within the question editor to expand the textbox width to allow for more space.

Inside of a summary report we do allow you to view data from a Table of Textboxes question type. However, a summary report is meant to provide aggregate data which with multiple rows of open ended data can be difficult to display in a table (as you have an infinite number of possibilities). The summary report will show you a count and percentage of how many times someone answered each particular cell in the table. For more specific data (such as the written responses), we would recommend viewing the CSV/EXCEL export.

The CSV/XLS export will assign a column within the spreadsheet for each row and column combination within the table of textboxes. You'll see above that it will first show you the Row Header, then the Column Header, and then the Table of Textboxes question itself. It will then display the respondent entered value in the column's cell.

**Note**: If a respondent has left a cell in the question blank, inside the CSV/EXCEL export, it will be represented by a blank cell as well.

File Upload

File Upload Questions are Available to Professional, SurveyGizmo, Enterprise, and Dedicated Accounts

The file upload question type allows you to have your respondents upload files and documents as part
of their survey response.

The file upload question type can accept multiple file uploads within a single question (maximum of 10), or limit the number of files that may be uploaded by the max number of files setting.

The uploaded files are attached to each individual response, available for download in bulk through  Project tab-->Export Data-->Bulk File Download. You can have them downloaded in a zip file in mass, but if you need to download them individually, you can do so on the monitor tab.

Custom Group

Custom Group Questions are Available to Professional, SurveyGizmo, Enterprise, and Dedicated Accounts

The custom group question type allows you to add different types of questions into one group of questions. For example, you could add a dropdown menu, an open textbox, and a multiple choice question all into one group of questions.

Contact Group

Contact Group Questions are Available to Professional, SurveyGizmo, Enterprise, and Dedicated Accounts

The contact group is a special question type that allows you to easily collect contact information from a survey respondent including, but not limited to: name, phone number, email address, and physical address.

 

This question type is generally used whenever you need to contact the person that filled out a survey after the fact.  An example of using this question type would be when you were creating a 'contact us' form to embed on your website. You may also want to use this option to find out if the survey taker has changed their address from what you have on file.

Custom Table

The custom table question type allows you to mix different question types into a single table. Questions types that can be added are: radio buttons, checkboxes, textboxes, essay boxes, and dropdown menus.

There can be a maximum of 10 different sub-questions in the Custom Table of any of the following question types. For each group, select the Question Type (1), enter the question title (2), and select whether it is required (3) and must be answered before the respondent can go to the next page. Additional groups can be added using the Add a New Column Group button at the bottom of the sub-question section.

Note: Answers can have the answer title and reporting value separated by the pipe symbol "|" if you wish to have a separate value for each.
