Vehicle Add/Change/Delete

Use this form to make an add or delete to the list of vehicles that your unit currently owns and to change budget numbers. Submit one form per vehicle. All university owned vehicles reported to Risk will have automatic liability coverage. Coverage ends or begins when the information is sent to the State of Idaho, Risk Management Program by UI Risk. To complete this form, you will need the vehicle identification number, year, make, and model. Please provide the license number if one has been issued. If the vehicle has special equipment, you will need to provide what the equipment is and the value associated.
