This eligibility survey is a screening tool that is used to determine eligibility for a larger research study. The purpose of the research study is to investigate the brain mechanisms that underlie memory, attention and perceptual processes. The study PI is Adam Gazzaley, M.D., Ph.D. from the UCSF Department of Neurology, Physiology and Psychiatry. In this eligibility survey we ask questions about your health, interests, education, etc. in order to better determine if you are eligible for the study. Please answer these questions honestly.

Study Title: A video game to enhance cognitive health in older adults, children, and adolescents

Participation in research is voluntary. You have the right to decline to participate in this online survey, or to withdraw from it at any point without penalty.

You will not be compensated for your time on this survey. In total, your time on this survey should not exceed 30 minutes.

If you choose to participate in this study, your survey data will be recorded. Be assured that your privacy is a priority to us. Your responses are kept on our secure servers and in secure databases. Please note that we may contact you in the future and invite you to participate in further studies if you are eligible.
