S14 Student Change Maker Award Nomination

Civics Day Student Change-Maker Awards Overview

In the spirit of celebrating the successes and achievements of our diverse stakeholders, Generation Citizen wants to recognize one outstanding college student, teacher, and student change-maker at Civics Day. We need your help in identifying them!

Thank you for taking the time to share information about an outstanding Generation Citizen student with whom you've worked. Specifically, we're looking for GC students who meet the following criteria:

*Identified a focus issue that is specific, relevant to students, and of manageable size
*Articulated a root cause that is a main reason the focus issue exists and is at a service or policy level
*Can clearly articulate shared goal that is logical, feasible, and closely tied to root cause
*Created an Action Plan to achieve their goal that engages, a decision-maker and at least one influencer an appropriate tactic
*Is implementing their Action Plan and moving towards achieving their goal or is reflective on challenges and ways to make a new course of action
*Worked collaboratively with classmates

Please feel free to nominate one or more outstanding individual. If you'd like to nominate more than one person for the same award, you should complete this survey then re-enter the survey link in your browser.

Please note a nomination does not guarantee that an individual will receive an award. All winners will be announced prior to Civics Day.

By submitting this form, you give permission for the story to be used at Civics Day, in promotional material, advertising material, including, but not limited to, reports, posters and electronic media.

Thank you for taking the time to recognize the incredible change-makers in our Generation Citizen community!
