Please complete and submit this application and non-refundable application fee ($10 for Oak Park-based organizations and $25 for out-of-district organizations). Give care to answering each section of the application and to providing the required documentation, as incomplete submissions may affect an organization's acceptance into the program and to the level to which they are classified.

Applicants have the option of saving the progress of their application using the button at the top of each page (starting on Page 2) and returning to complete it at a later date (up until the application deadline of October 7, 2016).

This is the person who will receive communication regarding PACT meetings, deadlines, updates, and other matters. *This question is required.
Please enter the contact information for the person who will be allowed to submit requests for use of Park District facilities on behalf of your organization for the upcoming year if your group is placed in the PACT Program. Requests will not be accepted from or discussed with anyone else in your organization. *This question is required.
