Purpose of Descriptor Questions for Exceptional Behaviors & Characteristics Questionnaire and Indicators of Gifted Behaviors Checklist

The purpose of the following questions is to get school input on ways that the student exhibits outstanding potential. This form is part of a multiple measures approach and will complement other sources of information including report card grades to determine if the student will benefit from placement in a district gifted program.  Questions are derived from the literature on gifted behaviors and characteristics. Your input is invaluable in making appropriate placement decisions to gifted and talented programs. This form should be completed by the student’s classroom teacher with input from other teachers who work with the child. Principals are asked to review it prior to submission.
