LAUSD & ALVS Enrollment Request Form

Please use the form below to request an Apex Learning VIRTUAL Course license. This form is to be completed ONLY by the site administrator or student's counselor. Complete this form for each student requesting a course.

Per REF-5670.2 the following defines Virtual Courses:

Course offered online with an online instructor provided by the vendor. These courses are intended for students needing original credit but are unable to include a specific class into their regular schedule. These courses can also be offered as a credit recovery option for eleventh and twelfth grade students who have been identified for NCAA eligibility. Although these courses provide an online teacher with the appropriate subject credential, the school must identify a credentialed teacher (in any subject area) to serve as a mentor for students. These courses are NCAA approved. Some online courses require materials to be purchased.

Virtual Course licenses CANNOT be reused or transferred to another student.

(Don't use the confirmation link from a prior enrollment when you need to add a new enrollment for an existing student. Only use the emailed link to return to an enrollment when you need to change that existing enrollment.)

6. Mentor(School Staff Only)
A mentor is required for all students enrolled in the Apex Learning Virtual School. Mentors will be enrolled in the courses they will mentor and have a version of the Grade Book designed specifically for their role in supporting student learning.

A coach is an adult such as a parent, guardian, or guidance counselor who receives weekly reports about a student’s progress. Coaches do not have accounts and cannot sign in to Apex Learning. Instead, coaches are indicated in student account profiles and receive reports via email.
