For these next questions, we want you to think about how active your child has been during the past two weeks. Think if he/she has been in school and been in gym class or running during recess. If your child is on a sports team, have they had any practice or games? Or has he/she been playing outside or done activities with their friends or family?
Now, think about how many times your child did the following types of activities for at
least 15 minutes at a time in the past two weeks.


Strenuous exercises are ones that your heart is beating fast or you can feel your heart
beating. Activities that are strenuous could include things like: running, jogging, hockey,
football, soccer, squash, basketball, cross country skiing, judo, roller skating, vigorous
swimming like on a swim team, vigorous long distance bicycling.

Moderate exercises are not exhausting. They don't take as much effort. Activities that are moderate could include things like: fast walking for exercise, baseball, tennis, easy bicycling or biking on a stationery bike, volleyball, badminton, easy swimming, down-hill skiing, dancing or dance team.

Mild exercises are ones that take minimal effort. Activities that are mild could include things like yoga, archery, fishing, bowling, horseshoes, golf, snow mobiling, or going for an easy walk.
