For these next questions, we want you to think about how active you have been during the past two weeks. Think if you have been in school and been in gym class or running during recess. If you are on a sports team, have you had any practice or games? Or have you been playing outside or done activities with your friends or family?

Now, think about how many times you did the following types of activities for at least 15 minutes at a time.

Strenuous exercises are ones that your heart is beating fast or you can feel your heart beating. Activities that are strenuous could include things like: running, jogging, hockey, football, soccer, squash, basketball, cross country skiing, judo, rollerskating, vigorous swimming like on a swim team, vigorous long distance bicycling.

Moderate exercises are not exhausting. They don't take as much effort. Activities that are moderate could include things like: fast walking for exercise, baseball, tennis, easy bicycling or biking on a stationery bike, volleyball, badminton, easy swimming, down-hill skiing, dancing or dance team.

Mild exercises are ones that take minimal effort. Activities that are mild could include things like yoga, archery, fishing, bowling, horseshoes, golf, snow mobiling, or going for an easy walk.
