Team Functionality & Culture Survey -- DCO DARWIN

Hello & Welcome to the TFAC (Team Functionality & Culture) Survey that I said I'd be asking you to complete after our individual discussions and start-up workshops about teamworking in DCO Darwin recently.

An 'Introduction' document has already been emailed to you giving you some background to the Survey, its purpose and how to complete it.  We also overviewed the 10 TFAC Dimensions in our last workshop and Part 4 of your Guide has detailed descriptions of each. A summary is provided here.

• I’d like to imagine that everyone will be willing to complete the Survey since we are a relatively small group and every person's opinions and ratings are important for the Team as a whole and will influence the results.
• I'd like to be able to start collating results mid-November so your help in completing the TFACS by NOVEMBER 15 is much appreciated.

Framing your Responses

A reminder to please base your Survey responses on your personal experience of the team over the past 12 months or so. (If you're a newer arrival, then adjust for your timeframe with the team).

• Answer from your personal viewpoint, not thinking about what the team as a whole might say.
• It is your collective individual viewpoints that will give us a picture of the culture of the team.

There's a mix of rating scales, word choices and open questions to answer in your own words. 

• Comments in your own words provide more valuable feedback overall than any rating scales. 
• We encourage you to please be generous with sharing your views and impressions.
• Don’t worry about saying things ‘the right way’ – just get your thoughts down.
• Note format will do. Don't worry about grammar, punctuation or spelling.
• There's no word limit. Try to give at least 5-7 points for each text question.
• A separate line for each point or comment please -- use your 'Enter' key; don't worry about layout or word wrap.

Importantly – You don't have to complete all responses in one go.  You CAN save your progress and return to the Survey until fully completed.

• The estimated completion time is roughly 30-40 minutes.
• There is a bar prompt to Save and Return at the top of the screen – click and provide your email address to receive a link that will let you return to finish your partially completed responses another time.
• This way you can also go back and add examples as they come to mind, to help explain your ratings.
