User Testing Intake Questionnaire










 
WID regularly conducts user testing sessions in order to improve the accessibility of products and services for people with disabilities. If you are interested in participating, please, fill out the questionnaire below. Please, answer all questions to the best of your ability. It is important that we have as much information as possible in order to place you in the appropriate user testing session. 

If you qualify for an upcoming session, you will be notified by a WID staff member, who will discuss scheduling and compensation with you.
