This application is for prospective PACT members only. All returning organizations must fill out the PACT 2019 application located on the Park District Website - 2019 PACT Application for Returning Organizations.
Give care to answering each section of the application and to providing the required documentation, as incomplete submissions may affect an organization's acceptance into the program and to the level to which they are classified.
Applicants have the option of saving the progress of their application using the button at the top of each page (starting on Page 2) and returning to complete it at a later date (up until the application deadline of Tuesday, October 1, 2019.
PACT Applications require an annual fee of $15 for organizations based in Oak Park and $25 for outside organizations. Please send your check to Park District of Oak Park, 415 Lake Street, Oak Park, IL 60302 or call 708/725-2000 with your credit card information by the October 1, 2019 deadline
