Feeding Assistant Training Program Annual Report, F-62592 (02/2019)

In accordance with state and federal requirements, all approved feeding assistant training programs must submit an annual report to the Office of Caregiver Quality (OCQ). All feeding assistant training programs approved to operate for the period of January 1 through December 31, 2018, are required to provide OCQ with training and testing statistics and information, even if class was not conducted.
