The Ponca Tribe of Nebraska needs your help understanding how to better serve Ponca Elders (ages 55 years and older). Ponca Elders who reside within the Ponca Service Delivery Area are eligible to receive a $25 gift card for completing the survey.
The survey is confidential. Your identity will not be released to anyone without the written permission of you or your legal guardian. This survey will take approximately 24 minutes to complete.
