Your chiropractor will study this information and will be ready to delve deeper into your history in person.

Upon completion you will be redirected to make payment for your visit. If this does not happen, you can find the link to payment in your confirmation email.
In the event your assessment and payment are not received prior to attendance, it is unlikely we will be able to care for you properly as we will not have had enough time to review your case in detail. You may then be asked to reschedule your appointment.
