2018-19 CMD Seat Reservation Form

The Jersey Arts Cooperative Media Database is a project of the Jersey Arts Marketers (JAM) sponsored by the ArtPride New Jersey Foundation and New Jersey State Council on the Arts. Each seat in the cooperative media database includes access to the MyMediaInfo platform, a web-based PR solution designed to help you efficiently identify, reach and engage traditional and social media contacts. Eligibility Requirements

Organization must be registered and in good standing with the Jersey Arts Marketers (JAM) network to be eligible for participation.
Organization must be a current member of ArtPride New Jersey to qualify for the member rate listed below. Others will be billed at the nonmember rate listed.

Registration Deadline: Extended to Friday, August 17, 2018

Your seat in the Jersey Arts Cooperative Media Database includes the ability to:

Build, save and export updated media contact lists using the industry's most accurate database
- Contact profiles are reviewed for accuracy at least every 90 days and date stamped to show the last update
Search for editorial calendar opportunities and set email reminders for upcoming deadlines
Build and reach an extensive social media influencer list of contacts with the social media contacts database
- Engage top influencers in Twitter, blogs and forums
- Directly interact with Twitter/blogs through the platform
- Build lists, see recent tweets, follow, retweet, direct reply, etc.
Receive training/walkthrough of MyMediaInfo system
Receive free 24/7 client/customer support from MyMediaInfo

Terms of Agreement

Details regarding access to the NASDAQ/MyMediaInfo platform, training and customer support will be provided to registrants upon confirmation of seat availability.
Limited seats are available for the 2018-19 program year.
The program year runs from September 1, 2018 - August 31, 2019.

I am interested in reserving a seat in the Jersey Arts Cooperative Media Database project on behalf of my organization. I understand that the fee for a reserved seat is non-refundable and agree to pay this fee. Rates are based on membership in ArtPride New Jersey.

Conditions

Seats in the Jersey Arts Cooperative Media Database are limited and available on a first-come, first-served basis to New Jersey-based cultural organizations registered with the Jersey Arts Marketers (JAM) network. Organizations requesting a seat will be sent an email confirming availability with instructions for payment. Submitting this request does NOT guarantee inclusion in the program. DO NOT SEND PAYMENT UNTIL AVAILABILITY IS CONFIRMED.

Your participation in the program also includes your acceptance and adherence to any additional applicable terms and conditions of use made by MyMediaInfo.

Submitting this form represents evidence that you are empowered by the organization listed to act on its behalf in this matter. By submitting, you agree to abide by all terms set forth in this agreement upon confirmation the seat requested is available. Seats are non-transferable and cannot be brokered to another service or organization. Fees incurred in reserving seats are non-refundable. Once request is confirmed, the organization reserving a seat will be financially responsible and billed per the terms above. In the case of any dispute, the decision of ArtPride New Jersey Foundation will be final.
