CALL FOR LIVE ART AND PERFORMANCE
Thank you for submitting work to HUBweek's Call for Live Art and Performance.

Please review the full call before submitting your proposal. Full details of the call can be found at this link.

Please only submit one entry per project. Questions with an asterisk (*) are required. It is recommended that you write your answers in a separate document and then paste them into this survey when they are ready. The page will not auto-save your work if left idle.

Submissions are due by 11:59pm EST on July 15, 2018. 

To complete this submission, you will be required to attach the following on page #2:
A) 1-page budget
B) Tech rider
C) At least 1 and up to 10 supplemental materials (photos, videos, or audio files)

If your budget/tech rider/supplemental materials are not ready, you should prepare them before starting your submission here. Acceptable file types = png, gif, jpg, jpeg, doc, xls, docx, xlsx, pdf, txt, mov, mp3, mp4
