Internship vaccination Application Introduction: Thank you for your interest in interning with us. The applicant is required to have vaccinations in order to complete the application process. Below are a list of the vaccinations the applicant is required to receive (marked with a red asterisk) and the three that are highly encouraged.
• Vaccination card or blood test results indicating vaccination or immunity to the following infectious diseases.
- Please submit 2 weeks prior to start of rotation to avoid internship cancellation.
*Failure to have or recieve required vaccinations will result in the application being voided.
